Sharon J. Jones, M.D. reported a physical break-in at its San Pablo, CA office on March 20, 2015. Approximately 17 patient charts, one desktop computer, two laptops, and a server were stolen. The stolen devices contained password-protected files including patient names, addresses, dates of birth, and basic medical information from letters written since 2007. No SSNs or financial data were included. The incident was discovered on March 21, 2015. Police were notified, and security measures were enhanced.
