Breached information was located on a Network Server. Exposed ePHI included names, addresses, dates of birth, driver's license numbers, Social Security numbers, claims and financial information, and treatment information. OCR provided technical assistance during the investigation. The CE implemented additional technical and administrative safeguards in response.
