Orthopedic Associates of Hawaii (d/b/a Minimally Invasive Surgery of Hawaii) disclosed a ransomware incident in February 2021. Systems were encrypted between Feb 12-19, 2021. An unknown actor accessed systems and downloaded limited data, including names, DOBs, SSNs, and health info. The Practices restored systems, notified law enforcement and HHS, and offered credit monitoring. No evidence of misuse was found.

Orthopedic Associates of Hawaii reported to HHS on 2021-04-20 a Hacking/IT Incident (ransomware attack) affecting approximately 138,000 individuals. Breached information was located on a Network Server. Exposed ePHI included names, addresses, dates of birth, driver's license numbers, Social Security numbers, claims and financial information, and treatment information. OCR provided technical assistance during the investigation. The CE implemented additional technical and administrative safeguards in response.
