National Seating & Mobility, Inc. (TN, Healthcare Provider) reported to HHS on 2019-04-15 a Hacking/IT Incident affecting 3,800 individuals. Several employees were victims of an email phishing scheme that compromised ePHI including names, birthdates, Social Security numbers, addresses, diagnoses, and treatment information stored in Email. The CE notified HHS, affected individuals, and the media, and implemented additional safeguards and staff retraining. OCR provided technical assistance and obtained assurances of corrective action.
