East Bay Perinatal Medical Associates (EBPMA) reported to HHS OCR on 2015-07-29 an Unauthorized Access/Disclosure affecting 1,494 individuals. Breached information was located on a personal employee laptop. Berkeley Police Department contacted EBPMA with a listing of its patients found on the employee's personal laptop. PHI involved included names and dates of birth. EBPMA notified media, sent individual notices, and offered credit monitoring. OCR required staff PHI safeguarding training. EBPMA subsequently became part of UCSF Benioff Children's Physicians.

East Bay Perinatal Medical Associates notified California regulators that an employee's personal laptop, containing a 2012 patient list, was identified by Berkeley Police during an unrelated investigation. The list contained names and dates of birth for approximately 1,000+ individuals. No SSNs or financial data were exposed. The laptop was retained by police, and the data was deleted from the employee's drive. Kroll was engaged for identity theft protection services.
