Health Net of California and Health Net Life Insurance Company disclosed a data security breach involving protected health information (PHI) and personal identifiers. Between March 1 and March 12, 2019, a coding error in the mailing process caused letters containing member information (names, DOB, SSN fragments, PHI) to be mailed to incorrect addresses. The incident was discovered, the code corrected, and affected correspondence re-mailed after validation. No financial information was involved.
