The unauthorized access occurred between January 7 and January 25, 2021. Affected data included names, addresses, dates of birth, insurance ID numbers, and health information such as medical conditions and treatment details. Health Net activated its incident response plan, ceased using Accellion's services, and is providing one year of identity protection services.

Health Net of California and Health Net Life Insurance Company experienced a data breach involving protected health information (PHI) due to a coding error in their mailing process. Between March 1 and March 12, 2019, letters containing member names, health plan IDs, dependent information, dates of birth, and partial Social Security numbers were mailed to incorrect addresses. The company identified the root cause, corrected the code, implemented testing and checklists, and re-mailed the correspondence. No financial information was involved.
