SAY San Diego notified clients that paper files containing personal information (names, case numbers, service dates) for participants in its Dual Diagnosis youth program were inadvertently left in a filing cabinet during an office move in 2013. The files were discovered in 2017 when a citizen returned some found at a salvage store. Files from March and April 2013 remain unrecovered. The organization engaged Kroll for identity monitoring and implemented stricter record-handling procedures.

On October 27, 2017, the CE was notified that a citizen had returned paper files containing PHI found in a filing cabinet purchased from a salvage store. Files related to youth participants in a dual diagnosis program (Jan–Jun 2013) included clinical and demographic information. The CE notified individuals, media, and HHS, offered free identity monitoring, revised its PHI disposal policy, and provided additional workforce training following OCR investigation.
