A logbook containing PHI — including names, addresses, birthdates, diagnoses, and other clinical information — was found to be missing. Breached information was located on Paper/Films. Following the breach, the BA discontinued logbook use and retrained staff on PHI safeguarding. OCR obtained assurances that corrective actions were implemented.
