Access Health Care Physicians, LLC (FL) reported to HHS on 2016-07-19 a Theft affecting 2,500 individuals. On May 26, 2016, an intruder broke into a physician's locked office and pried open locked file cabinets containing patient financial and clinical records stored on Paper/Films. Exposed data included names, dates of birth, phone numbers, home addresses, diagnosis codes, Social Security numbers, and insurance information. The CE notified HHS, affected individuals, and the media. Remediation included re-securing the office, changing locks, installing alarm systems, and moving inactive records to offsite storage.
