Exposed data included names, dates of birth, phone numbers, home addresses, diagnosis codes, Social Security numbers, and insurance information. The CE notified HHS, affected individuals, and the media. Remediation included re-securing the office, changing locks, installing alarm systems, and moving inactive records to offsite storage.
