Preventice Services, LLC (TX) reported to HHS on 2016-12-07 an Unauthorized Access/Disclosure affecting approximately 6,800 individuals. Business associate Zirmed, Inc. erroneously mailed notices containing other patients' names and dates of service due to a programming error by its sub-contractor, Allison Payment Systems (APS). The CE worked with the BA to correct the error and add technical safeguards. OCR confirmed appropriate BA agreements were in place and that required breach notifications were completed, including substitute notice posted on CE's website.
