Sacopee Valley Personal Care Agency, a healthcare personal care services provider in Parsonsfield, ME, inadvertently disclosed the protected health information (PHI) of one individual on November 28, 2025. The breach was discovered on December 1, 2025. The agency contacted the unauthorized recipient to recover/destroy the information, notified HHS OCR and state DHHS, and provided additional HIPAA privacy training to involved staff. One Maine resident was affected.
