Greenwood Leflore Hospital (Greenwood, MS) reported to HHS on 2014-04-16 a Theft affecting 3,750 individuals. An ex-employee of a BA hired to recycle/destroy old x-ray films stole films containing patient names, dates of birth, and x-ray images. The individual had been terminated by the BA prior to the theft, was later arrested, convicted of petit larceny, and ordered to pay restitution. The CE filed a police report, pursued civil litigation, notified affected individuals and media, and terminated its relationship with the BA. OCR reviewed related Privacy and Security Rule policies and training documentation.
