Two remote workforce members processing lab requisitions were discovered to not be the persons the covered entity originally hired. Exposed PHI included names, addresses, dates of birth, Social Security numbers, and lab results. The CE notified HHS, affected individuals, and media, provided substitute notice, offered credit monitoring, and implemented additional safeguards.
