Cigna Dental reported an insider policy violation on March 23, 2012, where an employee emailed an unencrypted document containing customers' first names and Social Security Numbers to her home email and her son's email. The employee was terminated, the document was destroyed, and affected individuals were offered two years of free credit monitoring. The incident was reported to the California Attorney General.
