Oldendorf Medical Services, PLLC (NY) reported to HHS OCR on 2012-01-24 a Theft affecting 549 individuals. Two unencrypted laptops were stolen containing ePHI including names, dates of birth, diagnostic test results, and social security numbers. The covered entity filed a police report and, following OCR investigation, implemented physical security improvements (cameras, new door locks, keypad code changes) and encrypted all laptop computers.
