On January 20, 2015, an unencrypted laptop stolen from a workforce member's car exposed PHI including patients' names, addresses, dates of birth, diagnoses, conditions, and medications. The CE responded by encrypting devices, updating HIPAA policies, training staff, offering free identity theft protection, and establishing a breach helpline. OCR obtained corrective action assurances.
