Good Care Pediatric, LLP (NY) reported to HHS OCR on 2015-11-12 a Hacking/IT Incident affecting 2,300 individuals. A Trojan Horse virus compromised one desktop computer device, making patient billing files accessible by unauthorized individuals online from January 1 through April 3, 2014. ePHI exposed included patients' names, addresses, telephone numbers, dates of birth, and diagnosis codes. The CE remediated by shutting down external access, scanning all devices, changing passwords, encrypting billing files, and retraining staff. OCR provided technical assistance on risk analysis.
