Community Family Care Medical Group IPA, Inc. reported that at least two of its former contracted physicians improperly disclosed members' protected health information to competitor independent physicians associations. The disclosed information included names, addresses, dates of birth, Social Security numbers, insurance identification numbers, health insurance information, and treatment and diagnosis information. The breach affected 7,173 individuals. The Office for Civil Rights (OCR) provided technical assistance to the entity regarding its obligations under the Breach Notification Rule.
