Community Family Care Medical Group IPA, Inc. reported that at least two of its former contracted physicians improperly disclosed members' protected health information to competitor independent physicians associations. The disclosed information included names, addresses, dates of birth, Social Security numbers, insurance identification numbers, health insurance information, and treatment and diagnosis information.
