An unencrypted laptop was stolen containing ePHI including names, addresses, zip codes, dates of birth, social security numbers, claims information, and diagnosis codes. The CE filed a police report and, following OCR's investigation, encrypted the laptop's backup drive and trained all staff on encryption practices for personal computers and mobile devices.
