Oakland Family Services (MI), a healthcare provider, reported to HHS on 2015-09-09 a Hacking/IT Incident affecting 16,107 individuals. On July 14, 2015, an unauthorized individual remotely gained access to an employee's email account exposing PHI including clinical and demographic information. The CE terminated the hacker's access, implemented a new email solution with improved password protections, and trained employees on HIPAA and IT security.
