Riderwood Village (also identified as Riderwood Senior Living Community) reported to HHS on 2013-01-17 a theft affecting 3,230 individuals. Five laptops (four unencrypted) were stolen from the facility's physical therapy department in MD, exposing ePHI of 8,507 individuals including names, dates of birth, addresses, health plan ID numbers, and therapy treatment notes. The CE notified affected individuals, issued a press release, posted substitute notice, encrypted laptops, revised security procedures, retrained employees, and provided OCR with written assurance of corrective action.
