Ronald Schubert MD PLLC (WA) reported to HHS on 2013-11-26 a Theft affecting 950 individuals. A physician's car was broken into at a public non-work location and an unencrypted laptop stored under the seat was stolen. The ePHI on the laptop included addresses, birth dates, SSNs, and clinical information in password-protected EMR software. The CE filed a police report, notified partners, and issued breach notifications to HHS, individuals, and media. Post-breach, all devices were encrypted and a formal mobile device safeguard policy was implemented following OCR technical assistance. Breached information located on Laptop.
