Sutter Medical Foundation reported to HHS on 2017-11-22 a Theft affecting 1,303 individuals. An encrypted laptop and a binder of paper documents were stolen from an employee's locked vehicle. The paper documents were subsequently recovered intact. PHI exposed included names, dates of birth, ages, medical record numbers, primary care physicians, treating providers' names, appointment times, visit types, and/or needed accommodations of 613 individuals. Breached information located on Laptop and Paper/Films. The CE sanctioned the responsible workforce member, offered one year of identity theft protection, and retrained its workforce.
