On July 17, 2018, Ruben U. Carvajal, MD reported a hacking incident to HHS affecting 3,775 individuals. An unauthorized party gained access to the electronic medical record (EMR) system through remote access software on a desktop computer. The compromised information included names, addresses, dates of birth, Social Security numbers, and extensive protected health information (PHI). In response, the entity replaced the computer, installed a new EMR system, removed the remote access software, strengthened passwords, and offered one year of credit monitoring to affected individuals.
