SEIU Local 32BJ, District 36 Building Operators Welfare Trust Fund (Health Plan, PA) reported to HHS on 2018-12-21 an Unauthorized Access/Disclosure affecting 911 individuals. An employee of business associate Express Scripts inadvertently emailed PHI intended for the covered entity to another multi-employer health and welfare fund. Exposed data included names, telephone numbers, SSNs, dates of birth, and prescription information (via Email). The covered entity notified HHS, individuals, and the media; provided credit monitoring; and ceased business with Express Scripts. OCR reviewed the CE's risk analysis.
