Hal Meadows, M.D.

Dr. Hal Meadows reported that patient files containing personally identifiable information, including names, addresses, birth dates, telephone numbers, insurance numbers, treatment codes, and billing information, were unlawfully accessed on July 27, 2016. The incident was reported to law enforcement, and the FBI retained the computer for investigation. Dr. Meadows implemented heightened procedures, safeguards, and staff training to prevent recurrence.
