The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE reset all passwords, implemented additional technical safeguards, and offered complimentary credit monitoring services to affected individuals.

Unauthorized access occurred on October 15, 2021, via a third-party medical provider's credentials. The intrusion was discovered on October 19, 2021. Exfiltrated data included PHI, SSNs, financial account info, and employee PII. The FBI and DOJ were notified; notification to residents was delayed per DOJ request until January 1, 2022. Remediation included password resets, MFA implementation, and third-party forensic investigation.
