North Broward Hospital District

North Broward Hospital District (operating as Broward Health) notified South Carolina residents of a cybersecurity incident involving unauthorized access to systems. The breach compromised personal, medical, and government identification data. Broward Health offered two years of Experian IdentityWorks and implemented enhanced security measures, including enterprise-wide password resets and multifactor authentication. The investigation was ongoing at the time of notification.

The covered entity (CE), North Broward Hospital District dba Broward Health, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 1,351,432 individuals. The PHI involved included names, addresses, telephone numbers, financial information, Social Security and drivers’ license numbers, health insurance information, diagnoses, medical record numbers, and email addresses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE reset all passwords, implemented additional technical safeguards, and offered complimentary credit monitoring services to affected individuals.

North Broward Hospital District reported a data breach to the Montana Attorney General. The breach was reported on 2022-01-01. The breach occurred on 10/15/2021. 140 Montana residents were affected.
