EyeCare of Bartlesville reported to HHS on 2015-03-13 a hacking incident involving a malware virus that affected 4,000 individuals. The malware locked an unencrypted server hard drive containing patient data, including names, addresses, dates of birth, insurance IDs, and diagnosis codes.
