Max M Bayard MD, PC (VT) reported to HHS on 2015-08-07 a Theft affecting 2,000 individuals. Two unencrypted laptops and one thumb drive were stolen during a burglary on August 5, 2015, containing ePHI of 2,154 individuals including names, dates of birth, insurance information, SSNs, dates/types of treatment, and diagnoses. Breached information located on Laptop and Other Portable Electronic Device. OCR investigated; CE implemented facility access controls, encryption, alarm system, and surveillance cameras.
