The logbook contained names and treatment dates of 822 patients. Breached information located on Other (paper logbook). CE revised policies, re-trained staff, eliminated the paper log book, and issued breach notifications to HHS, affected individuals, and a local newspaper. OCR obtained assurances of corrective action.
