An error in the Accela online application system deployed on August 8, 2015, made personal data (name, address, SSN, driver's license, phone, DOB) available to unauthorized users. Discovered August 1, 2016. No evidence of misuse. Remediation included vendor correction and offering 1-year Experian ProtectMyID.
