County of Sacramento Department of Human Assistance discovered on October 29, 2025, that an employee improperly forwarded work-related emails containing confidential client information (name, case number, DOB, address, SSN) to a personal email account. The County secured the accounts, revoked the employee's access, and is implementing technical controls and policy updates. Complimentary identity monitoring is offered to affected individuals.

County of Sacramento reported to HHS on 2023-05-23 a Hacking/IT Incident affecting 5525 individuals. Breached information located on Email. An employee was the victim of an email phishing attack affecting protected health information (PHI). The CE implemented additional administrative and technical safeguards and retrained staff on email security.

County of Sacramento notified residents that an error in the Accela software online application system may have exposed personal data (name, address, SSN, driver's license, phone, DOB) to EMS license applicants. The unauthorized access report was deployed on August 8, 2015, and shut off within an hour of discovery on August 1, 2016. The County corrected the issue with the vendor and offered one year of identity protection.
