Dean Health Plan (WI, Health Plan) reported to HHS on 2018-06-15 an Unauthorized Access/Disclosure affecting 1,311 individuals. On June 7, 2018, the CE discovered that due to human error it sent reminder letters to wrong addresses, exposing patients' names and their primary care facilities (PHI on Paper/Films). Staff were required to review document delivery policies. OCR obtained documented assurances of corrective action implementation.
