Dean Health Plan (WI, Health Plan) reported to HHS on 2018-06-15 an Unauthorized Access/Disclosure affecting 1,311 individuals. On June 7, 2018, the CE discovered that due to human error it sent reminder letters to wrong addresses, exposing patients' names and their primary care facilities (PHI on Paper/Films). Staff were required to review document delivery policies. OCR obtained documented assurances of corrective action implementation.

A mailing containing estimate of payment (EOP) documents was damaged in transit from business associate Emdeon to a bank via UPS. The USPS returned 31 of 148 pages to the CE on September 25, 2015. PHI exposed included dates of service, member names, health plan member ID numbers, and procedure codes. Breached information was located on Paper/Films. OCR investigated and reviewed BA agreement, UPS correspondence, and CE HIPAA policies.
