An employee lost a laptop containing PHI including names, addresses, Social Security numbers, dates of birth, dates of service, prescription information, and services provided for 1,994 individuals. Breached information was located on a Laptop. In response, the CE sanctioned the employee, notified authorities, and implemented remote-wipe software and full-disk encryption on all mobile devices. OCR obtained assurances of corrective actions.
