University of Oklahoma, Department of Obstetrics and Gynecology reported to HHS on 2015-07-03 a Theft affecting 7,693 individuals. An unencrypted, password-protected laptop was stolen from a resident physician's car. The laptop contained ePHI including names, dates of birth, medical procedure dates, medications, lab results, admission/discharge dates, treating physicians' names, and treatment plans.
