A CE administrative assistant provided a departing nurse practitioner with paper lists of patient names, addresses, gender, age, and dates of service in June 2013 and January 2014. Breached information was on paper/films. The CE terminated the employee, issued a cease-and-desist to the NP, destroyed the lists, and revised policies. OCR obtained corrective action assurances.
