Cosmetic Surgery Specialists, P.C. (PA) reported to HHS on 2019-07-23 an Improper Disposal breach affecting 3,000 individuals. A former business associate, Health Claim Services, left patient billing records in a storage facility; the records were placed in a dumpster after the storage space contents were auctioned off due to non-payment. Exposed PHI included names, home addresses, telephone numbers, health insurance information, and descriptions of services (Paper/Films). The CE arranged for the records to be shredded and notified HHS, affected individuals, and the media. OCR provided technical assistance regarding BA agreement retention requirements.
