Project Vida Health Center (PVHC) notified the New Hampshire Attorney General of a cybersecurity incident occurring on or around November 1, 2020. An unauthorized third party attempted to infiltrate PVHC's systems, potentially exposing patient names, addresses, phone numbers, and dental billing/treatment history. Only two New Hampshire residents were affected. Notifications were mailed on February 18, 2021, offering credit monitoring. PVHC engaged forensic specialists and enhanced security policies. No evidence of misuse was found.

The entity notified HHS, affected individuals, and the media; offered complimentary credit monitoring; performed a global password reset; removed compromised servers; and implemented additional technical safeguards.

Thieves bypassed security to steal encrypted servers containing ePHI including names, DOBs, SSNs, and addresses. The CE transitioned to a cloud-hosted system and implemented new access controls.
