On December 12, 2016, the Wonderful Center for Health Innovation discovered that a laptop containing patient medical information was stolen between December 9 and December 12, 2016. The laptop was not recovered. The device contained names, addresses, DOBs, SSNs, and medical records. The Center reported the theft to law enforcement and implemented remote-destroy software and additional safeguards. No unauthorized access was confirmed.

Wonderful Center for Health Innovation (CA) reported to HHS on 2017-01-20 a Theft affecting 3,358 individuals. A laptop was stolen from a workforce member's unlocked car between December 9–12, 2016. Breached ePHI included diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media, and implemented physical/technical safeguards, updated its security analysis, sanctioned workforce members, and trained all staff. OCR provided HIPAA Security Rule technical assistance.
