Baylor All Saints Medical Center at Fort Worth (TX) reported to HHS on 2013-08-05 an Unauthorized Access/Disclosure affecting 940 individuals. A former employee forwarded PHI via text messages from a CE pager, exposing patient names, demographic information, bed locations in the emergency department, and ER admission notifications. Breached information was on an Other Portable Electronic Device (pager). CE disabled pager copy-forward features, revised procedures, and OCR's investigation prompted vendor server configuration changes.
