Yakima Neighborhood Health Services (YNHS) notified the Washington AG that on October 4, 2022, a file containing patients' names, DOBs, medical record numbers, and treatment locations was inadvertently distributed to one individual. YNHS had the recipient delete the file. Notices were sent to 2,689 Washington residents on November 14, 2022. YNHS is implementing additional employee training and strengthening policies.

A workforce member inadvertently sent PHI — including names, dates of birth, other identifiers, and treatment-related information — to the wrong email recipient. The CE notified HHS, affected individuals, and the media, posted a substitute notice, and responded with additional training, technical safeguards, and revised policies.
