Jackson Therapy Partners, LLC inadvertently disclosed personal information of 401(k) plan participants via an unencrypted Excel file uploaded to Transamerica Retirement Services' Message Center. The file contained names, SSNs, addresses, dates of birth, and plan participation info. The error occurred on Nov 14, 2011, and was withdrawn by Nov 15. Only four account holders accessed the message, two of whom opened it. The company changed its data transmission method to prevent recurrence.
