Tom Wood East, Inc.

The covered entity (CE), Tom Wood East, Inc., reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 912 individuals. The ePHI involved included names, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained its staff on the proper methods of identifying and responding to fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
