Perry County Medical Center Inc. d/b/a Three Rivers Community Health Group (TN) reported to HHS on 2019-07-26 a Hacking/IT Incident (email phishing) affecting 3,812 individuals. An employee was victimized by a phishing scheme exposing ePHI including names, birthdates, clinical information, medications prescribed, and other treatment information via Email. The CE notified HHS, affected individuals, and the media, implemented new technical safeguards, and retrained staff on identifying fraudulent emails. OCR obtained assurances of corrective actions.
