Larsen Dental Care LLC (ID) reported to HHS on 2014-04-18 a Theft affecting 6,900 individuals. An unencrypted external hard drive was stolen from a workforce member's vehicle. Exposed ePHI included names, addresses, dates of birth, email addresses, phone numbers, dental records, medical history, health insurance numbers, and SSNs. The CE terminated the responsible employee, conducted a new risk assessment, encrypted all ePHI-bearing devices, and implemented new security/privacy policies. OCR obtained corrective-action assurances.
